IMPORTANT: Remote Services Self Certification for Vendors

In light of the current public health crisis, the Department of Developmental Services (DDS) has released directives that have included guidance related to the provision of services at alternate locations or via remote services. Please read the full directives posted on the DDS website. Eastern Los Angeles Regional Center (ELARC) is here to support our Service Providers to transition to a home-based, teleservices model. This situation presents us with a unique opportunity to come together in the support of our clients and the health of our communities. Through dedication, careful planning, and ingenuity we can continue to serve our valued clients.

ELARC has created a Self Certification Form for Service Providers. ELARC is requesting that all providers who are currently or will be providing remote services or services in an alternate location complete this form and return it to
