Parents, medical professionals, community agencies, local education agencies, social workers, nurses and others involved in a child’s care may refer a child to the Prevention Program. Referrals sent to the ELARC Prevention Program should include any available information including birth records, current medical records, developmental information, and family contact information.

Parents may contact the ELARC Prevention Program directly to refer their child.  Birth, medical, and developmental information is required to determine eligibility.
