Payment Schedule

Please process the Authorizations to Purchase of Service and Provider of Care Claim forms within the time frame. The properly filled and signed claim should reach us by the 3rd working day of the month for our first payment on the 11th and by the 15th of the month for our second payment on the 21st. We will try to meet these dates; delays of up to 3 days should be anticipated by you.

Payments for RESPITE SERVICE FAMILY MEMBER should have the properly filled out DS1811 form, and sent along with our Provider of Care Claim form.

Person Center Thinking

Vendors! Parents! Consumers! Join Us for Person Centered Thinking Training: ELARC, in partnership with the Vendor Leadership Forum, is launching an ongoing Introduction to Person Centered Thinking training program. This four-part session will be hosted on a monthly basis, every Wednesday morning. There is a fee for vendors and community members (consumers and families may have fee waived upon request) Registration is required as space is limited.
